Care Planning and Admission Process

At Assist Care, we have a comprehensive care planning and admission process designed to ensure that each child's needs are fully understood and met from the moment they arrive.

Our Referral Process

Thorough review of referral information by the Service Managers and Planning Manager
Assessment of the child's needs against our service capabilities
Consideration of age, gender, safeguarding concerns, and background information

Upon Admission

A warm welcome and introduction to the home
Sensitivity to the child's needs, particularly in emergency placements
Assignment of a key worker to help settle the child
Explanation of house rules, routines, and the child's rights and responsibilities
Provision of a child's guide containing relevant information

Our 28-day Induction Programme Allows for

Completion of a Baseline Assessment
Evaluation of referral information
Testing and confirmation of risk assessments
Identification of the child's wishes, aspirations, and ambitions Following this, a formal review is held, and future plans, goals, and risk management plans are agreed upon. Every child will have a placement plan for permanence in place within 6 weeks of admission, outlining desired outcomes and expected actions from each agency involved in their care.

Transition Planning

At Assist Care, we recognise that the transition from our care to semi-independent living or alternative arrangements is a crucial phase in a child's journey. Our comprehensive transition plan is tailored to each individual's unique needs and aspirations, ensuring a smooth and successful progression.

Personalised Life Skills Training

Focusing on practical skills such as budgeting, cooking and personal hygiene.

Educational Support

Continued assistance with academic or vocational studies.

Emotional Support

Ongoing counselling and mental health support as needed.

Independent Living Preparation

Gradual increase in responsibilities and independence within the care home.
